Clinical Documentation Analyst

Summary:

Interact with staff and colleagues to assist with review and modifications of clinical documentation to ensure accurate depiction of all data.

Duties:

  • Interact with physicians, nursing staff, case management, medical records coding staff, or other compliance specialists to help facilitate modifications of clinical documentation as well as ensuring accurate depiction of the level of; clinical services, reason for admission, patient severity, risk of mortality and conditions present on admission. 
  • Review the quality of medical record documentation and convey deficiencies to house staff and the attending physician(s).
  • Record chart findings in a dedicated documentation database on a daily basis.
  • Communicates with and educates members of the patient care team on an ongoing basis.
  • Participate in select committees and educational programs as necessary.

Education and Experience:

  • 3+ years of clinical experience in an acute care setting
  • BSN or related field required 
  • Must be currently licensed and/or registered as one of the following;
    • Registered Professional Nurse
    • Advanced Practice Nurse
    • Physician Assistant
    • Medical Doctor or other licensed health care professional highly preferred
  • Must be Certified as one of the following;
    • Clinical Coding Specialist (CCS)
    • Certified Clinical Documentation Specialist (CCDS)
    • Clinical Documentation Improvement Practitioner (CDIP)
  • Must have demonstrated skill in interacting courteously, diplomatically, and effectively with all members of a healthcare team
